Your Initial Visit
Paperwork
Before your first visit we do require some information about you and your health. There are 3 possible ways you can accomplish this.
1. You may submit the Registration and Medical History information online via our secure website. These forms will be automatically submitted to us, and this is the most popular method as it saves time on the day of your visit and allows you to make the most out of your consultation time with us. To complete forms online, you will need a user name and password to enter our secure site. You can either request an appointment online to have this information emailed directly to you, or please call our office at 425-450-0880 to have a user name and password generated for you.
2. You may download and print paper copies of the registration and medical history forms and complete them. You can either bring the completed forms with you on your visit or mail to us ahead of time.
3. You may use the traditional option of completing the registration and medical history information in our office on the day of your visit. If you choose this option please plan to arrive 15 minutes early to allow sufficient time for this.

Parking
There is underground parking and validation is provided for the duration of your office visits and procedures. When you enter the parking garage, please follow the signs marked "West Building" and enter through the green elevator bank, up to the first floor. As the parking garage gets very busy during the day, please proceed directly to the lower levels (P3 and P4) for open parking spaces.
Consultation fee
There is a $75 fee for cosmetic surgery consultations with our surgeons. This fee is fully credited toward any cosmetic surgical procedure you choose to have. Medical and Reconstructive consultations are billable to insurance.
For cosmetic surgery procedures you also have the option of having a no-charge consultation with one of our experienced patient care coordinators who can provide you with a great deal of information about the procedures, show you hundreds of before and after photos, provide you with cosmetic surgery price quotes, and if you so desire may take your photos to discuss with the surgeons.
Consultations with our non-surgical providers for injections and non-surgical procedures are complimentary as well. When scheduling a cosmetic consultation with our surgeons, you may give us your Credit Card information when you schedule the visit or, alternatively, you may use the Paypal link below to pay for the appointment at any time before scheduling.

Cancellation policy
We respect the value of your time and ask the same of you. Therefore, we request the courtesy of a 48 hour advanced notice for rescheduling or canceling your appointment. If this request is not honored, we reserve the right to ask for pre-payment of your fee by credit card prior to rescheduling another appointment for you. Cosmetic surgery consultations require pre-payment of the $75 fee, and this fee is fully refundable with a 48 hour notice of rescheduling or cancellation.
Privacy
The privacy of your personal and medical information will be fully maintained by our office. Also, during the course of your surgical procedure and subsequent visits, we will arrange for your entry and exit through a private door to the office.
